Visit the Find A Doctor site and use the search options to select a physician.
On the left side of the physician detail page you'll see "Request an Appointment".
Request an appointment by Department / Division (any doctor)
Find the Department or Division you are looking for, and go to that page.
On the left side of that page you'll see "Request an Appointment".

A Medical Center representative will contact you during the next business day between 7 a.m. and 7 p.m. EST
