Los signos de mediastinitis abarcan sensibilidad de la pared torácica, supuración de la herida e inestabilidad de la pared torácica en pacientes que se han sometido a una cirugía reciente. Una radiografía puede mostrar el ensanchamiento del mediastino, causado por la acumulación de células inflamatorias dentro del tórax.

Una TC de tórax puede mostrar la mediastinitis con mayor detalle. Así mismo, el médico puede insertar una aguja en el área de la inflamación y extraer una muestra para enviarla a tinción de gram y cultivo con el fin de identificar la fuente de alguna infección.
