El diagnóstico se realiza sobre la base de signos y síntomas clínicos puesto que las radiografías generalmente son normales. A menudo, se presenta dolor o sensibilidad cuando se presiona ligeramente el tendón cerca del sitio en donde se fija al húmero, por encima de la parte exterior del codo.

También se presenta dolor cerca del codo cuando se extiende la muñeca (doblada hacia atrás como cuando se acelera una motocicleta) en contra de la resistencia.
