Es una acumulación de sangre en la superficie del cerebro.

Los hematomas subdurales son con mucha frecuencia el resultado de un traumatismo craneal grave y, cuando ocurren de esta manera, se les denomina hematomas subdurales "agudos", los cuales están entre los más letales de todos los traumatismos craneales. El sangrado llena el área cerebral rápidamente, dejando poco espacio para el cerebro, y están asociados con lesión cerebral.

Los hematomas subdurales también se pueden presentar después de un traumatismo craneal leve, especialmente en las personas de edad avanzada. Estos hematomas pasan inadvertidos por períodos de muchos días a semanas y se les denomina hematomas subdurales "crónicos". Durante un hematoma subdural, las pequeñas venas que corren entre la superficie del cerebro y su cubierta externa (la duramadre) se estiran y se rompen, permitiendo que la sangre se acumule. En las personas de edad avanzada, las venas a menudo ya se han estirado debido a la atrofia cerebral (encogimiento).

Algunos hematomas subdurales ocurren sin causa (espontáneamente).
