Estos síntomas empeoran en forma lenta y generalmente se sienten en ambos lados del cuerpo.

El médico llevará a cabo un examen físico, el cual por lo regular muestra debilidad muscular y alteración en la sensibilidad en ambos lados del cuerpo, especialmente en las piernas. Los reflejos a menudo son anormales y los músculos pueden desarrollar espasticidad. Se puede presentar una reducción en el sentido del tacto, en el dolor y en la temperatura.

Los cambios mentales van desde falta de memoria leve hasta la demencia completa o psicosis. Es poco común la demencia grave, pero en algunos casos es el síntoma inicial del trastorno.

Un examen ocular puede mostrar daño al nervio óptico, una afección llamada neuritis óptica. Los signos de una inflamación nerviosa se pueden ver durante un examen de la retina. Igualmente, puede haber respuestas anormales de las pupilas, disminución de la agudeza visual y otros cambios.

Los exámenes de sangre, incluyendo un hemograma completo, nivel de vitamina B12 o nivel de ácido metilmalónico en la sangre se utilizan para diagnosticar anemia o una deficiencia de vitamina B12.
