No existe una prueba de laboratorio para diagnosticar el temblor esencial. El diagnóstico se basa en la historia del paciente, en un examen físico que revela temblores cuando efectúa movimientos voluntarios y ninguna otra anomalía.

Sin embargo, se deben hacer pruebas para descartar otras causas de temblores tales como exceso de cafeína, tabaquismo, abstinencia de alcohol , consumo de ciertos medicamentos (algunos medicamento para el asma, ciertos antidepresivos, litio y algunos medicamentos anticonvulsivos), hipertiroidismo , feocromocitoma y enfermedad de Wilson.

Las exámenes o pruebas de laboratorio de rutina y los estudios imagenológicos del cerebro (como una TC de la cabeza, una IRM del cerebro y radiografías) u otros exámenes generalmente son normales.
