El examen puede indicar un compromiso de las cuatro extremidades (cuadriplejia espástica) o debilidad en cara, brazos y piernas (síndromes de las neuronas motoras superiores). Los reflejos pueden ser anormales. El examen del ojo puede mostrar una pérdida de control de los músculos oculares, en particular una parálisis del nervio craneal VI (ver mononeuropatía craneal tipo VI ).

Una IRM craneal muestra la anomalía en el tronco del encéfalo (puente de Varolio) y es el principal estudio diagnóstico.
