Las cefaleas en brotes pueden ser diagnosticadas por un médico tomando como base los síntomas y el examen físico. Es posible que haya que realizar exámenes para descartar otras causas de los síntomas. El examen generalmente no muestra efectos neurológicos específicos a excepción del síndrome de Horner ( párpado caído o pupila pequeña de un solo lado) durante el ataque que no está presente entre un episodio y otro. Esto puede evolucionar y convertirse en un síndrome de Horner persistente.

Se puede llevar a cabo una IRM de la cabeza para descartar otra patología.
