El diagnóstico de la fiebre recurrente se debe sospechar en personas que provienen de áreas de alto riesgo y que presentan episodios repetitivos de fiebre. Esto es particularmente válido si la fiebre está seguida de una etapa de "crisis" y si la persona puede haber estado expuesta a piojos o garrapatas de cuerpo blando.
