Es un daño a la médula espinal, debido a la formación de un área llena de líquido dentro de la misma médula.
La acumulación de líquido que se observa en la siringomielia puede ser el resultado de un traumatismo de la médula espinal, tumores de la misma médula o defectos congénitos (especialmente la "malformación de Chiari", en la cual parte del cerebro comprime la médula espinal en la base del cráneo).
La cavidad llena de líquido casi siempre comienza en el área del cuello. Se expande lentamente, ejerciendo presión sobre la médula espinal y causando daño en forma lenta.
