Este trastorno debe diferenciarse tanto de la esquizofrenia de la infancia como del trastorno generalizado del desarrollo (autismo).

El signo más importante de este trastorno es la pérdida de los hitos fundamentales del desarrollo. El niño tiende a tener un desarrollo normal a lo largo de la edad de 3 a 4 años y luego se presenta una pérdida gradual de estas capacidades previamente establecidas (ej.: habilidades linguísticas, motoras y sociales). El diagnóstico se hace generalmente con la pérdida de funcionamiento en al menos dos áreas descritas anteriormente.
