Incompatibilidad RH

Enfermedad hemolÃtica del neonato inducida por Rh; hidropesÃa fetal
Tratamiento:
Debido a que la incompatibilidad Rh es casi completamente prevenible con el uso de RhoGAM, la prevención sigue siendo el mejor tratamiento. El tratamiento del neonato ya afectado depende de la gravedad de la condición.
Leve:
- Hidratación agresiva
- Fototerapia usando luces de bilirrubina
HidropesÃa fetal:
- Amniocentesis para determinar la gravedad
- Transfusión fetal intrauterina
- Inducción temprana del parto
- Transfusión directa de concentrado de eritrocitos (compatibles con la sangre del bebé) y también exanguinotransfusión del neonato para extraerle la sangre que contiene los anticuerpos maternos que están destruyendo sus glóbulos rojos.
- Control de la insuficiencia congestiva y de la retención de lÃquidos.
Kernicterus:
- Exanguinotransfusion (pueden requerirse múltiples transfusiones)
- Fototerapia
Expectativas (pronóstico):
Para la incompatibilidad Rh leve se espera recuperación completa. Tanto la hidropesÃa fetal como el kernicterus representan condiciones extremas causadas por hemólisis . Los dos tienen pronósticos reservados. La hidropesÃa fetal tiene una alta tasa de mortalidad.
Complicaciones:
Las posibles complicaciones abarcan sÃndrome neurológico con deficiencia mental, trastorno del movimiento, pérdida de la audición, trastorno del habla y convulsiones
Situaciones que requieren asistencia médica:
Se debe consultar al médico si una mujer piensa o sabe que está embarazada y no se ha realizado un control médico.
