El cirujano hace una incisión en el abdomen y examina los órganos abdominales, mientras el paciente se encuentra profundamente dormido y sin sentir dolor (bajo anestesia general).

Se emplean diferentes tipos de incisiones dependiendo de la condición del paciente. Las incisiones más comunes son la incisión vertical en la línea media, las incisiones transversas de los cuadrantes inferior y superior derechos o los cuadrantes inferior y superior izquierdos. Se toman muestras de tejido (biopsia) para permitir que se analice el área afectada. Cuando el tratamiento está terminado, se cierra la incisión, ya sea con sutura o grapas de piel.
