Resident/Fellow Contract: An executed contract is required for all residents and fellows enrolled in post-graduate clinical training programs sponsored by the University of Maryland Medical Center. This form is also available through the Graduate Medical Education website at Residency Coordinator > Postmatch Processes > Paperwork Processes > Credentialing, Contracting, Licensing Timelines

Resident /Fellow Data File:  This file is to be completed by the Program Liaison (e.g., Residency Coordinator, Administrative Assistant, Program Director) to assure that all residents and fellows are appropriately credentialed. The  instructions for completing, along with the form are identified with this form and which are also available on the Graduate Medical Education website at Residency Coordinator > Postmatch Processes > Paperwork Processes > Credentialing, Contracting, Licensing Timelines
