Instructions for Completing the Application for Interventional Cardiovascular Fellowship Program.

Please Note: All positions for the Interventional Cardiovascular Fellowship have been filled for 2009. Applications are being accepted for 2010.

Please download*, complete and forward one copy of the application form*.

Note: We require certification via the 3 parts of the ABME exam or FLEX. The University of Maryland, Department of Medicine, mandates this for all fellowship applicants. Please give the appropriate dates and/or scores.

It is not necessary to supply documentary evidence for any information already covered on the application. Specifically, DO NOT send copies of ABIM certificates, licenses, diplomas or publication reprints.

Please attach your updated CV

Three letters of recommendation are required: each letter should specifically address your experience and skills in the Cardiac Catheterization Laboratory at your University Training Program. All letters of recommendation should be submitted in enclosed envelopes and included with your application materials.

Send a copy of your ECFMG certificate and current visa status, where appropriate.
