Case Management

All patients at the Maryland Heart Center will benefit from individualized case management services. Nurse case managers communicate with the physicians, multidisciplinary teams and insurers to develop a discharge plan geared toward each patient's needs.

Services include referrals for home care, inpatient and outpatient rehabilitation and assistance with ordering supplies and equipment needed at discharge.
