Important Controlled Dangerous Substance (CDS) Renewal Instructions

To ensure the expeditious processing of your CDS renewal, please refer to
these instructions when preparing your renewal
application.
- Send completed renewal application with the appropriate prorated renewal
fee based on the last digit of your CDS registration number in the envelope
provided. For a prorated fee please see table below. Make check or
money order payable to: DHMH – Drug Control.
Write your CDS number on payment. Do Not Send Cash.

- Even if there are no changes with your name or address, the information
below the perforated line (e.g. SS #, Maryland License #, DEA #) must be provided.
- Change of Name: A copy of your marriage certificate, divorce
decree, or court order that validates the name change must be attached to
your application.
- CRNP's, CNM's and PA-C's must provide a Maryland
business address in order to process the renewal application. Must
have an approved "Written Agreement" from MBON or
"Delegation Agreement" from BOP to prescribe controlled substances.
Please do not mail in your CDS application until
its approval since CDS applications can not be processed without an approved
"Written Agreement" or "Delegation Agreement".
- If you are filing "Exempt" status, a Maryland business
address must be provided.
- Check your application for completeness, have you:
- Completed all required information?
- Signed and dated the renewal application?
- Included a signed check or money order?
- Include a copy of your State of Maryland Professional License.
Please allow 3 weeks before inquiring about the status of your CDS certificate.
