Clinical Placement Forms and Information

Welcome to the University of Maryland Medical Center (UMMC). This site has been created to serve as the central resource for all levels of nursing students that are utilizing UMMC for school related training purposes. Please complete all necessary paperwork prior to state date. Additional policies and guidelines have been added for reference. All students should read these policies and are expected to adhere to these standards.

Required Paperwork and Information for all students coming to UMMC

Clinical Faculty and Student Orientation Manual

Parking information (Student Parking)
Badge Form – Come to the Security Office with your form between 12 p.m. and 3 p.m. if possible. Please fax completed forms to Security prior to arrival.

Clinical Groups – Nursing, EMT, Phlebotomy

Clinical Instructors: Please print one copy of the Orientation Manual Acknowledgement, Confidentiality Agreement and Department Orientation. All the students should sign the group form and fax it to Cyndy Ronald, 410-328-8258 or scan and send to cronald@umm.edu
