Orthopaedic Residency Program

University of Maryland Department of Orthopaedics Tumor Surgery Rotation- Alvin and Lois Lapidus Cancer Institute Sinai Hospital of Baltimore

PGY 3 Goals and Objectives

Patient Care:

Faculty will evaluate the resident's ability to obtain an H&P and order pertinent radiographic studies to obtain an appropriate diagnosis and treatment plan for outpatient and inpatient consults.

In the operating room the faculty will evaluate the resident's familiarity with the patient and their radiographic studies, the indications for surgery, understanding of the surgical treatment, ability to execute the surgical plan and understanding of the postoperative care.

The resident will:

  1. Effectively demonstrate that he/she can:
    1. Can obtain a comprehensive history of the tumor presentation
    2. Take and understand a complete history of previous illnesses
    3. Identify the pertinent radiographic studies which will help to delineate the nature of the tumor
    4. Interpret the appropriate radiographic studies by demonstrating an organized approach to evaluating the study
    5. Formulate a differential diagnosis based on the patient demographics, history physical examination and radiographic studies
    6. In patients without a known tumor diagnosis they will understand the principles of selecting the appropriate type of biopsy
    7. In patients with a known diagnosis and metastatic disease they will demonstrate an understanding of the importance of multidisciplinary care
    8. They will understanding the principles in selecting non-operative  versus operative care for metastatic disease
  2. Effectively be able to understand and describe their role in the treatment of the following conditions
    1. Benign bone tumors of the extremities and spine
    2. Benign soft tissue tumors
    3. Primary malignant bone tumors
    4. Primary malignant soft tissue tumors
    5. Metastatic disease of the spine pelvis and extremities
  3. Effectively communicate which conditions they can treat effectively and which require referral to a tertiary facility and at what state in the workup or treatment that referral should be made.
  4. The resident will demonstrate mastery of needle biopsy and open biopsy technique
  5. The resident will demonstrate mastery of prophylactic fixation of long bone pathologic fractures and impending lesions

Medical Knowledge:

Faculty will evaluate the resident's knowledge on an ongoing basis in the clinic and operating room
The resident will:

  1. demonstrate knowledge of the clinical behavior of primary and metastatic tumors
  2. understand the prognosis  of common primary and metastatic tumors
  3. demonstrate and understanding the relevant anatomy and pathophysiology of primary and metastatic tumors
  4. demonstrate understanding the evidence supporting current treatment methods for primary and metastatic tumors

Practice based learning and improvement

Residents will demonstrate and understanding of the recent advances in the filed of tumor treatment and participate in the multidisciplinary bone tumor conference
They will:

  1. acquire an understanding of the importance of prebiopsy review of the diagnostic materials as compared to the review of the biopsy specimen. This will help in honing the skills for achieving and accurate differential diagnosis for tumors and prevent errors in diagnosis
  2. acquire an understanding the difficulties in reading pathologic material and the necessity for review of prior material for errors in diagnosis

Interpersonal and Communication Skills

The resident will see and experience multidisciplinary care of tumor patients. They will acquire an understanding the of methods of communication that are necessary in complex tumor patients to achieve optimal care.
