Referrals
We welcome physician referrals for patients who have acute or chronic pain. We request that all pertinent records (history and physical, radiologic, psychologic and other consultations) be forwarded to our Center prior to the initial appointment with the patient. Payment
We participate with Maryland Blue Shield, Medicare and many HMOs. As a courtesy to patients, the Pain Center is prepared to submit claims to private insurance carriers. Before an appointment may be scheduled, patients are requested to contact their insurance company regarding coverage and to bring to their first visit the appropriate authorization and/or referral form.
