Visiting Nurse Association
Visiting Nurse Association (VNA) of Maryland has served the community
since 1895 and has a well-established reputation for excellence in home care
and hospice. As a component of the University of Maryland Post-Acute Network,
referring patients for home care services has never been easier.
The VNA offers on-site Home Care Coordinators (HCC) to assist physicians in
evaluating and arranging home care or hospice services for hospital and clinic
patients. HCCs can be accessed through University Case Managers on each nursing
unit and are available 8:00 a.m. - 4:00 p.m., six days (Monday - Saturday a
week. You may also contact the VNA directly through the main number, for community
referrals.
Admission Criteria
Because each insurer varies, utilizing the HCC is the best way to evaluate
patient eligibility. General guidelines are as follows:
- Home Care: must be under a physician's care, need
a skilled (not custodial) intermittent service and be homebound.
- Hospice: must be under a physician's care, have
decided that they want a hospice approach to care and have a prognosis of
six months or less if the disease takes its normal course.
