Perioperative Medication Orders for Solid Organ Transplants

Type of Transplant: Kidney and pancreas, pancreas alone (PTA)
Kidney Source: (SPK, Cadaver donor, SPLK: Living donor)
Immunosuppression Protocol: Campath
Orders: Preoperative/Intraoperative

Preoperative

No immunosuppression medication given

Intraoperative

Immunosuppression

  1. Campath 30 mg IV over 2 hours
  2. Solumedrol 500 mg IV

Antibiotic

- Administered before skin incision
- Redose after 3 hours of surgery
(No penicillin allergy)

  1. Piperacillin Sodium / Tazobactam Sodium (Zosyn) 2.25 gram IV
  2. Gentamicin 1.7 mg/kg IBW IV

OR
(Penicillin allergy but able to take cephalosporins)

  1. Cefoxitin 1 gm IV
  2. Gentamicin 1.7 mg/kg 1BW 1V

OR
(Penicillin allergy but not able to take cephalosporins)

  1. Ciprofloxacin 400 mg IV then 200 mg IV after 3 hrs of surgery
  2. Clindamycin 900 mg IV
  3. Gentamicin 1.7 mg/kg IBW

Other: Anticoagulation for PTA (non uremic)

  1. Heparin IV bag 25,000 U in 250 ml D5W (to be sent to operating room from pharmacy), 200-300 units/hour.
