In order for us to make your first visit as productive as possible, please arrive 15 minutes prior to your scheduled appointment, and bring the following items with you:

New Patient Forms - If time allows, these forms will be mailed to your home. Should you not receive them prior to your appointment, click here to print forms * New Patient Forms*
Insurance Card(s)
Referral (if required by your insurance company)
Co-payment
Current list of any medications you take, including dosages. Be sure to include all vitamins, herbs, and any other complimentary alternative medications.
Any medical records you may have such as prior hospitalizations, X-ray reports, cath reports or films, and lab reports, etc.
