Residency Program

Rotations: University of Maryland Medical Center (UMMC) - Obstetrics

Educational Purpose: The University of Maryland Obstetrics rotation provides the resident with a fundamental knowledge base as well as clinical experience and competence in the area of obstetrics.

The OB team consists of a Chief Resident, a PGY-3, a PGY-2 and an intern. In addition there is a Family Medicine intern who participates as an equivalent OB-intern.

PGY-1 - Intern

Educational Objectives:

Develop ability to manage labor and delivery including:
- Appropriate triage of patients
- Working knowledge and understanding of the diagnosis and treatment plan for each patient on the ward
Demonstrate ability to perform safe and expeditious vaginal delivery
Demonstrate ability to perform postpartum bilateral tubal ligation
Demonstrate ability to perform neonatal circumcision for male infants
Recognize deviations from normal in antepartum, intrapartum and postpartum periods
Understand the options for obstetric analgesia and anesthesia and their possible respective maternal and fetal complications
Perform and interpret antepartum and intrapartum fetal assessments including basic ultrasound, biophysical profile, fetal monitoring, fetal scalp sampling
Provide postpartum care for vaginal and operative deliveries
Learn basic principles, diagnosis and management of common obstetrical disorders (e.g. preterm labor, preterm rupture of membranes, pre-eclampsia, placenta previa, placental abruption, postpartum hemorrhage, intra-amniotic infection, endometritis)

Demonstrated Competencies:

Circumcision
Management of 1st, 2nd, and 3rd stage of labor
Vaginal delivery
Episiotomy repair
Basic ultrasound
Biophysical profile
Fetal monitoring
Postpartum care
Abdominal tubal ligation

PGY-2

Educational Objectives:

Continue to enhance experience and improve skills listed in PGY-1 objectives
Demonstrate competence in performing primary or repeat cesarean section with assistance
Demonstrate competence in performance of emergency cesarean section
Develop the ability to manage the ambulatory and inpatient care of patients with complicated pregnancies (e.g. HIV, diabetes, hypertension ,multiple gestation, thyroid disease, seizure disorders, hemoglobinopathies)
Provide appropriate counseling to patients with advanced maternal age or who are at risk for genetic disorders
Demonstrate competence in performance of level-2 ultrasound
Ability to perform 2nd trimester amniocentesis

Demonstrated Competencies:

Cesarean delivery (primary, repeat, emergency)
Postpartum care
Antepartum testing
Preterm labor
PROM
Antepartum care of OB patients with medical complications
Level II ultrasound
Amniocentesis

PGY-3

Educational Objectives:

Continue to enhance experience and improve skills listed in PGY-2 objectives
Demonstrate ability to independently perform a primary or repeat cesarean section with supervision
Demonstrate competency in the management the ambulatory and inpatient care of patients with complicated pregnancies (e.g. HIV, diabetes, hypertension ,multiple gestation, thyroid disease, seizure disorders, hemoglobinopathies)
Understand the indications, contraindications and principles of, and be able to perform more advanced obstetric procedures (e.g. operative vaginal deliveries, vaginal breech deliveries and breech extractions, repair of difficult genital tract lacerations, management of shoulder dystocia)

Demonstrated Competencies:

Cesarean delivery, primary, repeat, emergent
Evaluation and management of high risk obstetric patients in-patient and out-patient
Advanced obstetric procedures
Obstetric emergencies

PGY-4 - Chief Resident

Educational Objectives:

Continue to enhance experience and improve skills listed in PGY-3 objectives
Demonstrate competence in managing an OB service at all levels of care including:
- primary and repeat cesarean sections
- postpartum tubal ligation
- repair of episiotomy (1st-4th degree)
- 3rd trimester amniocentesis
- external cephalic version
- management of normal and abnormal labor and delivery
- fetal scalp pH
- simple and complicated operative vaginal delivery
- management of shoulder dystocia
- repair of cervical lacerations and vaginal sulcus tears
- primary and repeat cesarean sections
- Emergency cesarean section
- postpartum tubal ligation
- management of labor and delivery of multiple gestations
- management of postpartum hemorrhage (including uterine artery ligation, postpartum/cesarean hysterectomy)
Function in a supervisory capacity assisting junior residents in the care of obstetrical (high and low risk) patients
